Monetize Videos With the New YouTube Video Edit Page

Are you having problems monetizing your videos with the new interface?
Are you unsure how to use the video edit page?
What happens if YouTube does not approve your video? What happens if monetization is denied?
OK, I made a video about monetization back on April 3rd. What to write in the additional info box when you are submitting a video. Typical YouTube, they have changed the monetization process.
